We use the following:
Concurrent disk replacement in an SSA RAID Array.
This procedure assumes you have already taken the necessary steps for backup and recovery, before starting the disk replacement. Depending upon the failure, additional steps may be necessary. Once you have determined the pdisk is part of a RAID Array, perform the following steps.
Logically remove the disk from the array:
(If the drive has already been removed and the status of the array is "degraded," skip to the next section.
1. Fast path to SSA RAID smit menu: smitty ssaraid
2. Select Change Member Disks in an SSA RAID Array.
3. Select Remove a Disk from an SSA RAID Array.
4. Select the RAID Array from which you a removing the disk drive (more than one may be listed).
5. Press F4 to select the Disk to Remove. From the displayed list, select the disk drive that is to be removed.
6. Enter to remove the disk drive from the array.
Set Service Mode on the disk to be physically removed:
1. enter 'diag' on the command line to enter diagnostics mode.
2. Select Task Selection (Service Aids)
3. Select SSA Service Aids (At the bottom of the selections)
4. Select Set Service Mode
5. Select the appropriate pdisk to remove.
6. Select Set or Reset Service Mode. (Once selected, and > will appear next to the pdisk on the screen. This indicates the pdisk is in Service Mode). If more than one disk is to be removed, do this procedure one at a time. You can also identify the disk from this screen. This will flash the check light on the drive.
7. The Service Personnel (C.E.) may now replace the disk drive.
8. Reset Service Mode to normal.
Add replaced disk to the RAID Array:
1. Remove the old pdisk number (drive which was replaced): rmdev -dl pdisk#
2. Issue cfgmgr -v from the command line to re-configure the newly replaced disk. The pdisk just removed in step with re-acquire with the new drive. A new AIX system disk (hdisk) will now be available.
3. Fast path to SSA RAID smit menu: smitty ssaraid
4. Select Change/Show Use of an SSA Physical Disk and select the appropriate adapter (There may be more than one SSA adapter in the system)
5. Select the appropriate pdisk from the correct SSA loop.
6. Change the current use from AIX System Disk to Array Candidate Disk.
7. Select Change Member Disks in an SSA RAID Array and Add a Disk to an SSA RAID Array. Select the new replaced pdisk.
May be somewhat anal, but works for us.

If it is a RAID array, the disk should already be in 'Rejected' phase.
Just rmdev the pdisk and add the new one. run cfgmgr on ssar and set it up
as a hot spare.

Folks,
I have an IBM SSA 160 array that has a bad disk
Could someone point me to the correct procedure for replacement?
(IBM will replace the drive)
