I'm also working on this with IBM - ANY ideas welcome.
We upgraded a large, busy system from 4.3.3 to 5.2ML3 about two weeks
ago. Now, on 4 separate occasions, we have received

We run qmail. This filesystem is on a LV which is on SSA disks using SSA
RAID. It's big - about 150GB. This always happens while the system is
under heavy load.
This has obviously been quite a panic, and has kept me up all night
several nights waiting for downtime so I can boot into single user mode
and run fsck on it. Mostly, fsck does not find any problems.
IBM has had me updating bos.rte.<several things> to 5.2.0.40, and
updating the microcode in the SSA adapter cards to the latest, to no
avail.
Anybody seen this before or have any idea what is going on?
