If the mirroring is correct, then indeed everything should continue as
normal.
My guess is that there's an unmirrored paging space: either hd6 or some
additional paging space. If that were the case I would expect the system to
keep running initially but any process that tried to access data on the
missing paging space would fail.
The other possibility that occurs to me is if the mirrored disks are using
the same I/O or bus or SCSI adapter, and either the failure is actually on
that or the disk failure has caused an additional problem - e.g. with SCSI
bus termination - leading to subsequent I/O failures.

This mornng I heard (second hand) that one of their SAP application servers (M80 running AIX 4.3.3 - not sure what ML) had a disk failure on one of the mirrored SCSI disks in rootvg. All other VGs are on a SAN. They then had to reboot the server, because the application processes starting failing. Sorry this is a bit vague, I do not have access to these boxes. Just wondered if anyone else had any experience of this. We have never lost a rootvg SCSI disk on any of the boxes supported by our team, but I would expect everything to continue and just hot swap the disk as we do with SSA disks.
