try to make the boot image manually:

# bosboot -ad /dev/hdisk0

... and see if you get the same error? If you don't then it's the tape.
Try setting the tape to "0" - variable block size and try rewinding the
tape
before attempting to perfrom the mksysb: mt -f rewind.

Anyone see this one:

0512-016 mksysb: Attempt to create a bootable tape failed:
bosboot -d /dev/rmt0.1 -a failed with return code 45.

mksysb RC is 3
Error running mksysb of rootvg. Aborting!

......

And it is running now....
