HACMP failed over on loss of mirrored rootvg

Folks,
We had an AIX 5.3 server running HACMP failover when one of the two
mirrored rootvg disks failed. We would not see that as reason to failover
since one drive was still alive and well.
What did we set wrong? Did we have a quorum setting wrong? Or is this
working as designed?
Let me add that we have one LV that is not mirrored, it is the sysdump LV.
We have the Primary on hdisk0 and the Secondary on hdisk1. It was hdisk1
that failed.
Also, on the same day we setup mirrored rootvg on this system, we did five
other LPARS and they are all set to NO for Quorum, so I don't think this
one would have been Yes.... But you can't be sure.
