After replacing the disk, this always works for us:
1. Remove the old pdisk number (drive which was replaced): rmdev -dl pdisk#
2. Issue cfgmgr -v from the command line to re-configure the newly replaced disk. The pdisk just removed in step with re-acquire with the new drive. A new AIX system disk (hdisk) will now be available.
3. Fast path to SSA RAID smit menu: smitty ssaraid
4. Select Change/Show Use of an SSA Physical Disk and select the appropriate adapter (There may be more than one SSA adapter in the system)
5. Select the appropriate pdisk from the correct SSA loop.
6. Change the current use from AIX System Disk to Array Candidate Disk.
7. Select Change Member Disks in an SSA RAID Array and Add a Disk to an SSA RAID Array. Select the new replaced pdisk.

Think back to the days of SSA haha...
I have a SSA RAID-5 array with no hot spare. pdisk0 died. I set it to
servicemode, then physically replaced it before doing an rmdev. Now I
can't get the new disk defined. It shows up as defined, but when i try to
mkdev i get the below error. Perhaps remnants of the old disk hanging out
somewhere? The diag link verification show a break in the loop where
pdisk0 is. The array is still up, just degraded. Any advice on how to
prceed is appreciated.
# lsdev -Cl pdisk*
