Have you tried replacing/checking the cables? Does it always happen to ad4?It happens to both drives and to both drives I replaced a month ago with these. Didn't replace the cables back then, but they were correctly attached and are now. Also it would be odd that both cables are broken at the same time.
Your drive could be dying, try swapping it out and see if the errors
persist.
