Mirroring problems

Dear managers,
I am trying to re mirror a v100 after replacing the failed disk. I did:
1. Installed the backup copies of vfstab and system files taken before
the mirror.
2. Boot the system from non-mirrored file system
3. I have tried removed the metadb, cleared the stripes and detached the
mirrors for making way for new mirror.
Now when I run metadb I get :
# metadb
flags first blk block count
a m pc luo 1050 1034 /dev/dsk/
a luo 2084 1034 /dev/dsk/
and I tried to create new metadb and I get:
# metadb -a -f -c2 /dev/dsk/c0t2d0s4 /dev/dsk/c0t0d0s4
parse_ctd(): partial_ctd_str = malloc(-2)
I have searched the google and there was one similar prob. As per that I
removed the entries in /etc/lvm/mddb.cf but no success.
