Boot disk failed in VXVM4.0

Hi Gurus,
I ma having SF4800 with root disks mirrored using veritas vol.Manager
4.0.Now,my boot disk has failed and i had booted the system from the
mirror...when i replced the root disk with a new disk using vxdiskadm step 4
and step 5 i am getting the following errors for step 5.Has any body
encountered such error..Pls let me know...
