How dangerous would it be to change the driver to generate the page up/page down instead of up/down arrow key events ? (by dangerous, I mean if one coudl see user interface issues).

The impression I have so far (this is my first time using a computer with a working thumbwheel) is that moving just one line up/down doesn't seem to provide "action" to make it really that useful.
