I tried the address of descriptor for 6 byte item with
short id = 4 and long value = 20

Hmm, it's difficult to determine which IP stack you are using. First you are looking in the TCP/IP Services examples, then you quote from a TCPware manual... I'm assuming it's TCP/IP Services, and not Process Software's TCPware.

You need to look at the TCP$C_TCP_KEEPINIT argument for a IO$_SETMODE call to $QIO(W). It's documented here:
