The easiest setup would be the DEC Office cable BC16E-xx and the MMJ (Modified Modular Jack = 6 pin tab offset) to D sub 9 adapter H8571-J.
Another possibility would be the BN24H-xx cable which one end has the RJ45 connector and the other the MMJ. Then you need an adapter which converts the RJ45 to D sub 9.
