SCSI card crashes SRM

I have a Qlogic ISP1020 SCSI card that I bought from a DEC hardware
vendor to use in my PC164LX. SRM works fine without the card plugged
in, and crashes with something bad address with the card plugged in.
I've tried various PCI slot combinations between the SCSI card and my
VGA card, always crashes.
Is this a known issue? Where can I find more errata and knowledgebase
about DEC hardware, HP has been difficult especially with older
hardware.
