It's good that it's solved!
Can you describe what controllers are in the machine? If there aren't
as many as there are numbers, then something is causing there to be
holes in the allocation. I'm fairly sure I understand *why* this
happens - the machine is trying to `leave space' for a controller
which has been temporarily removed so when it comes back the devices
on it will have their usual names, but it's somehow got confused about
controller identities (or maybe you've genuinely unplugged some
controller and chucked it away or something!). What I can't remember
is the mechanism that does this - I found it once when something like
this happened to us, but I forgot to write it down like a fool. Does
anyone else remember how it's done?
