The barcode is giving the part number 5012741114581.
From the numbers I guess it's an option number X1032A, part number501-2741 Single-Ended Ultra/Wide SCSI/FastEthernet (SunSwift PCI).
There may have been two versions of the card and Yours would then be
version 1.0, whatever that means. Version 2.0 would be part number
501-5656.
