Here's the relevant bit from my xorg.conf;
Section "Device"
   Identifier "RADEON-9000"
   Driver "radeon"
   BoardName "Radeon 9000"
   BusID "PCI:1:0:0"
   ChipId 0x514D
   #VideoRam 131072
EndSection

This works for both xorg and XFree86 4.2.x on both FreeBSD and NetBSD
