It's possible that the 5.0.5 lsil driver doesn't fully support your adapter.
But first, try to make things as simple as possible: Remove or disable
in the system BIOS the 21040 adapter which uses the slha driver.
Remove any other PCI cards other than video.

Boot from the 5.0.5 install CD and watch the hardware inventory screen
for any significant pauses as the drivers test themselves against the
hardware. In particular, the blc driver sometimes causes problem and
has to be disabled in the bootstring.

Write down all the info for the %adapter line(s) and post it here,
along with any driver error messages.

Also, make sure that the hard disk is plugged into the first bus,
not the 2nd. Simply plug the cable into the other channel
and see if the BIOS now reports it any differently.

And of course, check the basics. Is the disk drive bus properly
terminated with an external terminator?
