Veritas disk replacement problems (A1000)

First the setup: Two A1000 disk arrays with four disks in each. Each
A1000 has 2 LUNS (a pair of two disk stripe sets) Under Veritas, LUN 0
from first A1000 mirrored to LUN 0 from second A1000, same for LUN 1.
A drive in LUN 1 started getting r/w errors and then RAID Manager
failed that physical drive so LUN 1 went from optimal to dead. For
some reason, LUN 0 also went from Optimal to Dead. Veritas then failed
the Veritas disks on each mirror. The mirrors stayed running from the
other A1000.
This is what I did to replace the drive. I physically replaced the
drive in the A1000. It spun up fine, green light came on. Opened RM6,
both LUNs changed to optimal. On system removed both Veritas disks
(app1, data1), option #4 from vxdiskadm. Ran format to see if solaris
has access to LUNs that just changed to optimal. Drives came up fine
in format partition tables correct. Vxdisk list shows veritas app1 and
data1 as "removed" and cXtXdX as error. Ran vxdctl enable, vxdisk list
shows cXtXdX as online, no name, no group. I then chose option 5 on
vxdiskadm (replace a removed disk), chose app1 to replace, but got
error that there were no available drives to use as replacement. Do I
need to reinitialize the two disks, option 1 vxdiskadm or vxdisk -i ?
I did try to run a vxdisk -i on one of the disks and it complained that
it was already a part of a disk group and I would need to use -f to
force an init.
I do not have direct access to the system right now but can get any
outputs requested in short time.
