If this is a mail server, the following is off base.
Is there a /etc/mail/submit.cf.old ?
diff /etc/mail/submit.cf.old /etc/mail/submit.cf might be the answer.
I have a subnet that just runs the sendmail client with
D{MTAHost}[mailserver IP] in /etc/mail/submit.cf
The patch has D{MTAHost}[127.0.0.1]
