cannot install Digital Unix 4.0A

Hi,

I am trying to install Digital Unix 4.0A on a LX164

  i am using a SCSI cd-rom a scsi disk 4.3G WD

 The SCSI controller is a KZPBA-CX (recognised by the SRM)
 The Video card is a S3 board(recognised by the SRM)

  it is a 21164 with 128 MB of memory with SRM 5.8

  when i boot from the CDROM (DKA200) the system boots ,identifying
the hardware assigning rz2 to the disk and rz4 to the cdrom, it
detects the video card and the lasst message i see is
kernel console trio0

after that the screen clears and the system crash CP SAVE_TERM halt=1

There is no other message, anybody has an ides of what is going on?
